Functional Outcome After Mechanical Thrombectomy with or without Previous Thrombolysis

Introduction Combined intravenous therapy (IVT) and mechanical thrombectomy (MT) is the standard treatment for acute ischemic stroke (AIS) with large vessel occlusion (LVO). However, use of IVT before MT recently being questioned. Objectives To compare patients treated those alone, in a real-world scenario. Methods Retrospective analysis AIS LVO anterior circulation who underwent MT, or without previous IVT, between 2016 2018. Results A total 524 were included (347 submitted to IVT+MT; 177 alone). No differences groups found except higher time from onset CT groin puncture group (297.5 min vs 115.0 394.0 250.0 respectively, p < 0.001). Multivariable showed that age<75 years (OR 2.65, 95% CI 1.71–4.07, 0.001), not using antiplatelet 1.93, 1.21–3.08, = 0.006), low prestroke mRS 4.33, 1.89–9.89, initial NIHSS 0.89, 0.86–0.93, absent cerebral edema 7.83, 3.31–18.51, mTICI 2b/3 4.56, 2.17–9.59, 0.001) independently associated good outcome (mRS 0-2).
